In today's social and cultural climate, many LGBTQ+ youth and adults continue to navigate heightened experiences of stress, uncertainty, discrimination, and identity-based stigma. While we cannot change every challenge they encounter, we can change what happens when they encounter us. Research consistently shows the profound impact that even one caring, affirming adult can have on the well-being of LGBTQ+ young people. Feeling seen, respected, and supported by a trusted adult is associated with better mental health, increased resilience, and reduced risk for depression and suicide. While this finding has been highlighted among youth, the need for affirming relationships extends across the lifespan. Adults also benefit from environments where they are welcomed, respected, and valued for who they are. For therapists, this may mean consistently communicating openhearted curiosity, unconditional positive regard, and affirmation throughout treatment. You might simply say: "I know there's a lot happening in the world right now that may affect LGBTQ+ people in different ways. I don't want to assume how you're feeling, but I want you to know this is a place where we can talk about it if it ever feels important. And regardless of what's happening outside these walls, you don't have to carry it alone." Affirmation is not reserved for the therapy room. Every interaction—whether with a receptionist, case manager, physician, teacher, supervisor, probation officer, behavioral health technician, volunteer, or administrative staff member—communicates something about whether a person belongs and is safe. For caregivers, it may mean being the person who celebrates a youth's identity, listens with love, and creates a home where authenticity is welcomed. As a caregiver, you might say: "I know there has been a lot in the news lately about LGBTQ+ people, and I imagine some of it might feel confusing, frustrating, or even scary at times. I just want you to know that I love you exactly as you are. If you ever want to talk, ask questions, vent, or just sit together, I'm here. You don't have to figure it out by yourself." For organizations, it means recognizing that LGBTQ+ people experience your culture long before they experience your clinical expertise. An affirming organization isn't created by one highly skilled clinician. It is built through hundreds of everyday interactions in which every staff member contributes to an environment of dignity, belonging, and emotional safety. This Month's Reflection Ask yourself: If someone walked through our doors today, what would communicate that they are safe, welcomed, and valued before the first clinical conversation even begins?
